Dear LANP Member:

My name is Tonya Fowler and I am a Family Nurse Practitioner graduate student.  I am writing to invite you to participate in a research study to be conducted in partial fulfillment of the requirements for a Master of Science in Nursing at Southeastern Louisiana University.

This survey will be conducted using Survey Monkey and is designed to gather information from nurse practitioners who see newborn infants in the outpatient setting within their first four weeks of life. Many changes in practice have been implemented regarding newborn umbilical care and I will be examining the type of cord care provided at birth and at home, and the condition of the umbilical cord site as seen by the practitioner in the outpatient setting.

This is a voluntary survey.  By completing this survey, you are acknowledging informed consent for participating.  This survey consists of 15 multiple choice questions and will only take 15 minutes of your time.  Participants do not have to answer every question, and may quit at any time, but responses to all questions would be greatly appreciated.  There are no risks associated with completing this survey except for the usual mild discomfort associated with completing a survey.  There are no benefits associated with completing this survey except that the data collected may add to the body of knowledge regarding newborn care. Participant responses will be kept anonymous.  The participant's name will not be provided to the researcher or associated with any survey answer.  Please click the link below to access the survey.
