This is a courtesy email on behalf of the Louisiana Department of Health in regards to the Small Town Health Professionals Tax credit. The application period will open September 1st 2019 and close October 31st 2019. A brief explanation of the program is detailed below, please open the attachments listed in the email for more information. 

 SMALL TOWN HEALTH PROFESSIONALS TAX CREDIT ABOUT   

 The Details:

Who: Rural primary care practitioners, including: Family Physicians, General practice physicians, Internal Medicine, Pediatrics, Obstetrics/Gynecology physicians, primary care Nurses practitioners, primary care Physician Assistants, Dentists and optometrists.

 What: The Small Town Health Professionals credit allows approved applicants to receive a nonrefundable tax credit in the mount of $3,600 unless subject to proration

 When: For 2019, this program accepts applications from September 1st- October 31st. No late applications will be accepted.

 Where: Must practice in a rural area as defined in rules promulgated by the Department of Health and be located in a Health Professional Shortage (HPSA) designated area.  Please visit: https://www.ruralhealthinfo.org/am-i-rural to determine whether the practice location is in a Rural and HPSA area.

 How to Apply:

Please note that any applications received prior to September 1st will not be accepted. Make sure to have the following documents and information available (Once the application is started, it has to be completed; there is no save option):

  • Last 4 digits of your Social Security Number
  • Scanned copy of your current medical license
  • Medical License Number
  • NPI Number
  • Medicaid/Medicare Numbers
  • Saved “Am I Rural” report (#2 below)
  • Patient Information
  • How many patients to you examine daily/annually
    • Total # of Medicaid Patients
    • Total # of Medicare Patients
    • Total Number of Uninsured Patients
    • Total Number of Sliding Fee Scale/Indigent Patients
