To Whom It May Concern,

My name is James Jeane, I am currently pursuing my Masters Degree in Nursing at McNeese State University with a Family Nurse Practitioner concentration. I am conducting a research project under the tutelage of McNeese State University nursing faculty. The task and proposed purpose of this survey is to assess Louisiana Nurse Practitioner’s knowledge of managing otalgia. I am asking you to complete a short research survey with a link provided below. The survey consists of questions with regards to your perception of the incorporation of 2% aqueous lidocaine in the management of otalgia, traditional otalgia management techniques, and a few demographic questions. Completion of this survey should take ten minutes or less to complete. Participation is completely voluntary with no incentives or associated retribution. You can stop this survey at any time with your responses being anonymous and confidential. All responses will be reported as aggregate data with no identifying information shared with third parties. Your anonymity is protected through transport layer security, secure connections, encrypted data transmission, public key cryptography, and message integrity. The allotted time frame for completion of this analytical survey is from 11/14/2017 to 11/28/2017.
