Statewide Survey of Screening and Prevention Efforts

From the Louisiana Department of Health, Medicaid Quality Improvement Section:
I invite you to participate in our statewide survey on screening and prevention efforts. As we continue to develop new strategies for how to reach and provide effective, quality care to Louisiana's population, it is important for us to know what efforts are already in place.
We hope to receive responses from across the state and from across a variety of clinical and public health entities. We encourage you to share this survey with your colleagues.
The survey will take approximately 15 minutes to complete. Your responses are voluntary and confidential. Only aggregate data will be reported. Please complete the survey by Friday August 4th.
