Position Description
Job Summary: Affinity Health Group is looking to fill positions for Nurse Practitioner's in the Monroe / Shreveport and Alexandria areas. The Population Health Nurse Practitioner (NP) will perform a wide variety of duties in the Population Health Program. The primary focus of the NP is to assist in improving overall quality of care of patients while meeting the standards of care identified through HEDIS, Medicare, and other accredited agencies by performing in-home, in-office, or telemed Population Health Visits. Main components of the program include, but are not limited to, collaborating with the deemed Health Maintenance Organization (HMO) to evaluate the member’s health risk, providing case management, updating preventive services, and providing accurately diagnostic evaluations of the HMO members. Population Health Department also offers a quarterly incentive bonus plan.
Essential Duties & Responsibilities:
- Coordinate with other members of the Population Health team and Health management department to help develop, implement, and expand on the program
- Perform detailed HRA visits on members of the Health Maintenance Organization in an in-home or in-office setting as outlined in the Population Health Nurse Practitioner Visit Policy
- Perform preventive testing such as IRIS Diabetic eye exams, HgbA1C, Urine Microalbumin, Peripheral Vascular Disease testing, and other testing deemed appropriate during visit
- Initiate and facilitate individualized follow up care to the member between the HMO, Health Management team, the patient, and the primary care provider
- Effective communication between all teams is essential
- Provide accurate documentation and diagnosing based on diagnostic criteria set forth by multiple healthcare agencies
- Provide education to the patient regarding importance of preventive screening and other healthcare needs that may arise with the visit
- Contribute to organizational growth initiatives for the Health Maintenance Organization
- Remain current on reimbursement and program rules and regulations, industry trends, and development of concepts and procedures
- Perform chart reviews for retrospective review for risk adjustment
- Provide provider education on wellness coupons, wellness billing, and diagnostic coding
- Ensures patient and personnel confidentiality
- Comply with OSHA guidelines noted by Affinity Health Group’s policy and procedure manual as applicable
- May be asked to participate in health fairs within the Health Maintenance Organization or Affinity Health Group
- Performs other duties as assigned
Licenses, certifications, and/or registrations:
- Current State Registered Nursing License & APRN license (NP)
- Current Driver’s License
- Proof of current automobile liability insurance

Supervisory Responsibilities:
- Works with the accompanying team member during home or office visits and provides supervision when needed in relation to the visit coordination
Supervision Received:
- Works under supervision of the Population Health Manager Supervisor and Manager
Working Conditions:
- Travel will be required
